CSTEM

The Core Specialist Training in Emergency Medicine (CSTEM) is a professional development course spanning three years, during which individuals occupy various posts at non-consultant hospital doctor (NCHD) level. This programme serves as the fundamental stepping stone for prospective specialisation in this field of medicine.

The CSTEM rotations are predetermined and, whenever feasible, they take place within a single hospital or geographic area.

After completing their intern year satisfactorily, doctors are qualified to apply for entry into CSTEM.

During CSTEM, you will obtain a wide range of experience in a variety of specialities, structured as six-month posts.

Year 1

  • Emergency medicine

  • Acute medicine, trauma and orthopaedic surgery, or plastic surgery

Year 2

  • Paediatric emergency medicine or acute general paediatrics emergency medicine

  • Emergency medicine

Year 3

  • Anaesthesia/critical care

  • Emergency medicine

Throughout your training, you will work towards successfully completing the Intermediate Fellowship Exam of the Royal College of Emergency Medicine (FRCEM) of the Royal College of Emergency Medicine/MRCEM.

To progress to ASTEM:

After finishing CSTEM and succeeding in passing the MRCEM intermediate exam you will obtain your CCCT and meet the qualifications necessary for applying to partake in the ASAT process (Assessment of Suitability for Advanced Training).

Collaborating with the HSE National Doctors Training and Planning (NDTP), the programme's goal is to advance CSTEM trainees who are considered qualified for further training. However, each year there is a limited number of advanced SpR posts available. In case the demand from CSTEM candidates exceeds this set limit, competition will be implemented in allocating these positions.

ASTEM

ASTEM, which stands for Advanced Specialist Training in Emergency Medicine, is a comprehensive four-year training program designed to help aspiring emergency medicine professionals progress further in their career.

To be eligible for the program, you need to possess a minimum of three years of clinical experience after completing your internship in the following areas:

- Emergency medicine experience in Ireland, the UK, South Africa or Australasia must be at least 18 months.

- You must have either completed a minimum of six months in anaesthesia/intensive care medicine, or demonstrate proficiency in CSTEM competencies related to this area via workplace assessments. The relevant areas include airway management, rapid sequence intubation, organ support, central venous cannulation and arterial cannulation.

- A requirement of at least six months' experience in paediatric emergency medicine or acute paediatrics.

- A duration of 12 to 18 months in emergency medicine or related specialties such as orthopedics, surgery, and internal medicine.

- ASTEM requires all experience to have been gained within five years preceding your application.
